Individualized care and support to achieve personal goals
Care management is central to JFCS’ Disabilities Services because we believe that addressing each person’s unique needs effectively requires considerable care, thought, and coordination. Through our comprehensive care management services, we support adults with disabilities and their families and caregivers with a personalized assessment, an individualized care plan, and ongoing support to achieve their goals.
Our caring and experienced care managers provide expert evaluations, help problem-solve, and coordinate services to suit each individual’s circumstances. Our aim, as always, is to help our clients achieve and maintain optimal levels of well-being and self-sufficiency.
Special Needs Trusts
Alongside day-to-day care coordination, JFCS supports families in preparing for their disabled child’s future when they are no longer able to do so. Our unique Special Needs Trusts program combines both Fiduciary Services and Disability Services which work in tandem to prepare for the future.
A Special Needs Trust is a legal document that protects specific assets of an individual with a disability, while retaining eligibility for needed governmental benefits for healthcare and financial assistance. JFCS may be named as a Trustee or Successor Trustee for a Special Needs Trust, and acts on behalf of the beneficiary to administer the Special Needs Trust.
In addition, JFCS’ Care Managers meet regularly with the adult child and their family to build a personal relationship and become familiar with the family’s needs and support systems so that we can step in seamlessly when needed.
