Since 1999, our L’Chaim Adult Day Health Care Center has provided outstanding medical, rehabilitative, and social services to thousands of elderly and disabled clients. The center serves Russian emigre clients from San Francisco and northern San Mateo county.
At L’Chaim, we offer a variety of services under one roof, specially tailored to meet individuals’ and families’ needs in a warm, supportive, and friendly atmosphere. Our staff of professionals provide skilled nursing and personal care; case management; psychiatric consultation, physical, occupational, massage, and speech therapy; nutritional counseling; and therapeutic and socialization activities. Transportation, hot lunch, and snacks are also provided.
L’Chaim Adult Day Health Care is a licensed facility and outpatient center that provides medical, social and therapy services for disabled adults (18 and older) and elderly persons with a range of health conditions. The program is a person-centered, long term care home-based community support program that aims to keep people living in their community.
In person-centered care, the participant is at the center of the care provided by the staff. This personalized approach is client-directed so that the participant’s values, preferences, and goals are integrated into their care plan and daily program. Participant empowerment is fundamental to the person-centered care model.
Our goals are to maintain and enhance our clients’ health; help them to remain independent in their homes and community for as long as possible; and reduce the likelihood of their falling, visiting the emergency room, or remaining hospitalized for prolonged stays, and avoiding premature institutional care.
Building Community
L’Chaim Health Center hours are 9:00am – 1:00pm and 2:30 – 6:30pm, Monday through Friday. Staff is available 8:30am – 7:00pm on those days.
We accept Managed Care Medi-Cal participants and scholarships, subsidies, and private pay is available.
L’Chaim, located at 2534 Judah Street, is in the heart of San Francisco’s Sunset District. Our staff is bilingual in Russian and English.
