At one of the most difficult times for people, Seniors•At•Home can provide an interdisciplinary team of professionals who support the lives of those who are failing or dying with a plan and services that address medical, psychosocial and spiritual needs.

The Seniors•At•Home Palliative and End of Life Care Program is designed to relieve suffering and improve quality of life.

  • Provide assistance with relief of pain and stress at any stage of a serious illness
  • Support optimum quality of life for clients and families
  • Address medical, emotional, social and spiritual needs
  • Coordinate care among health care providers and care settings throughout the course of an illness

  • Chaplain visits
  • 24-hour on-call nurses
  • Palliative Care consultation services to address any questions and concerns and help you make informed decisions about this challenging time in life

  • A coordinated team of professionals and volunteers, including nursing specialists, care managers, clergy, caregivers, and a consulting physician
  • Works closely with your physician and other healthcare providers
  • Coordinates hospice services if and when chosen by you
